Process for Issuing Rebaone Health Cards
Registration
Lifestyle & Wellness members register for the Rebaone Health Card via an online portal, clinic, or toll-free service, providing basic personal details (e.g., name, contact info, ID number).
Medical Screening & Data Entry
Members complete a required medical screening at a Plexilife EMR-integrated clinic, where providers capture health data (e.g., history, vitals, prescriptions) into the system for centralized storage and personalized care planning.
Card Processing
The clinic secures the member’s medical data via the advanced EMR, facilitating the Rebaone Health Card’s development—emergency-ready with a unique ID for seamless record sharing with providers.
Issuance
The card is printed and issued within two months of registration, with members notified via toll-free service or email for pickup or delivery, enabling centralized, efficient healthcare access
